Here’s how

Begin the process by filling out the questionnaire that applies to the type of involvement you are seeking, either help for the family, as part of a small group, or in the form of individual spiritual direction. Once you have completed the questionnaire, press the send button. Your information will be reviewed and Renee Farkas, director of HomeLife Initiatives, will contact you regarding the next step. In most cases the next step will involve an in-home meeting oriented around information gathering and assessment.

HomeLife Initiatives is highly individualized and the process will reflect your needs and desires. In the case of family and individual services, an Individualized Learning Plan (ILP) will be written and personalized support will be provided as you implement the ILP. If you are seeking involvement in a small group, Renee will facilitate by scheduling the time and place, providing pertinent resources, and leading the group process.
